CSM STUDENT POLICIES AGREEMENT

 In order for you to have the best experience at Cincinnati School of Music (CSM), we ask that students and their parents (for students under 18) read, understand, and adhere to the following guidelines:

Tuition and Automatic Payments

All lesson tuition payments are due in advance on a monthly basis and are processed automatically through Cincinnati School of Music’s secure payment processor (Mindbody Software). On the 1st day of each month, that month’s tuition is automatically charged to the credit card or bank account on file for each student. Tuition is not prorated, credited, or refunded for lessons missed or cancelled by the student. For students who enroll for lessons in the middle of a month, the first month’s tuition is prorated according to the number of scheduled lessons in that first month, followed by full monthly tuition starting on the 1st of the subsequent month of lessons. Monthly tuition does not include registration fees, any retail or teaching materials such as books or practice journals. There is a non-refundable registration fee of $45.00 per student upon enrollment or re-enrollment.

Variable Monthly Tuition

Monthly tuition is charged according to the number of scheduled weeks in that month. For example, in months that have four weeks of scheduled lessons (with one lesson per week), you are charged for 4 lessons. In months that have 5 weeks of scheduled lessons (with one lesson per week), you are charged for 5 lessons. In months that have 3 weeks of scheduled lessons (with one lesson per week) due to a scheduled holiday closure, you are charged for 3 lessons. 

CSM is not responsible for students’ weekly lesson attendance. Tuition is never refunded, credited, or prorated for any lessons that are missed or cancelled by the student or their caregiver.

Teacher Cancellations and Substitute Teachers

If your teacher has to miss a teaching day, a qualified substitute teacher on staff at CSM will teach the student’s regularly scheduled lesson. If a substitute teacher cannot be arranged, the student can reschedule this lesson through the office with any teacher on staff.

Student Cancellations and Rescheduling with a Minimum of 24 Hours Notice

Students who provide the CSM Office with a minimum of 24 hours written notice of a cancellation have the option for a courtesy private makeup lesson. All makeup lessons are based on teacher availability. At certain times of year, available times may be limited. As such, we do not guarantee a makeup lesson for every missed lesson by the student. CSM does not refund or offer tuition credits for any lessons missed by the student, whether or not they cancel with advanced notice. All notices of cancellations and requests for makeup lessons must be directed to the CSM Office, not your teacher. When the office receives advanced notice of a cancellation, we will take that lesson off the schedule and make that time available for other students to schedule a makeup lesson. All makeup options expire upon discontinuation of lessons.

School Closing/Delayed Openings Due To Weather or Other Circumstances

Occasionally, CSM must close early, close the whole day, or have a delayed opening due to inclement weather, a power outage in the area, or other hazardous conditions. For inclement weather such as snowy or icy conditions, we closely follow weather reports and make a judgment call as to the best plan for the safety of our staff, teachers, and students. We will make every effort to announce any weather closure or delayed opening with as much notice as possible via email, and on our CSM Facebook page. Any affected lessons due to a weather closure or delayed opening will be rescheduled at a later date. We do not offer refunds or credits for these types of cancellations.

Tardiness

If a student is late to their lesson, the missed time is forfeited and will not be made up by the teacher. In the event that the teacher is late, the missed time will be made up at the end of the lesson, or at the student’s next lesson.

Discontinuing Lessons and Monthly Tuition Charges

15 days written notice is required to stop lessons and future payments. Students wishing to stop lessons and recurring charges to their credit or debit card must give the CSM Office (not your teacher) 15 days notice in writing. Students are responsible for paying lesson tuition for their final two weeks of lessons whether or not they attend those lessons. Lesson tuition is nonrefundable and can not be credited for lessons missed by the student. 

In order to stop tuition payments for the next month, you must provide the CSM Office with notice of your withdrawal by the 15th day of the current month. Withdrawal notices submitted after the 15th of the month may result in a prorated tuition charge for the following month, based on our 15-day notice policy. If written notice (email) is not given to the CSM Office for discontinuing lessons, the recurring monthly tuition may continue to be charged. 

CSM also reserves the right to discontinue any student’s lessons at any time, for any reason. These reasons include, but are not limited to, failure to pay tuition and being disrespectful or disruptive to our staff, teachers, or other students.

Scheduling Questions Must Go Through the CSM Office

All scheduling matters (including but not limited to student lesson cancellations, conflicts, vacation plans, requests for a different weekly lesson time, and discontinuing notice) are handled at the front desk through the CSM Office. Summer Lessons

One of the benefits of taking lessons with us is that we offer lessons year round, including throughout the summer. We understand some students may go out of town during the summer and may miss lessons due to vacations, travel plans, or camp. To continue reserving your weekly lesson time with your teacher, monthly tuition payments are required, as this serves to reserve your weekly lesson time. If you will miss certain weeks in the summer, you have the option to makeup these missed lessons if you provide at least 24 hours notice to the CSM Office of any lessons you will miss. Students who stop lessons and monthly payments during the summer give up their lesson time to the next student on our waiting list and will need to re-enroll when they resume lessons. We do not offer any “holds” of lesson times without tuition payment, which serves to reserve the lesson time with the school.

Care of Students

Cincinnati School of Music is not responsible for the care or safety of any student before or after that student’s scheduled lesson time. Parents and/or caregivers of students must either remain on premises during the lesson time or return before the end of the lesson to pick up their child. Parents and/or caregivers are responsible for the care of their children who are not in scheduled lessons while on CSM premises.  

Liability Waiver 

Cincinnati School of Music is not responsible for any injuries incurred while on Cincinnati School of Music premises. By signing the bottom of this form, the signed parent or adult student will not hold CSM and any of CSM’s Directors, Office Staff, Contracted Music Teachers, or Contracted Accompanists responsible in the event of an injury on CSM premises during or outside of scheduled lesson times. 

Media Consent and Image Release

We feature photos and/or videos of CSM students for our newsletters, promotional materials, articles, on the walls of our schools, and on our official CSM Facebook/Twitter/Instagram pages. We never identify with students by their full name unless given written consent from the student and or the student’s parent/guardian. This form grants us permission to use a photo or video of you or your child in our materials. If you do not wish for CSM to use an image of you or your child, please notify the CSM office in writing and we will respect your wishes. 

Lesson Observation

Parents are always welcome to sit in on their child’s lessons as long as the lesson is not being disrupted, and the teacher is given command over the lesson time.
